Health Disability Activity Pathway
The Health Disability Activity Pathway is an NHS-led initiative delivered in partnership with Disability Sport Wales and Local Authority Sport Development Teams (or equivalents) to help connect disabled people with opportunities to be physically active (including sport participation).
The pathway has been created to enable health and social care professionals to refer disabled people for signposting to opportunities in their local area.
You and your team can access training to learn more about the pathway and how to refer; please access the training tab and get in touch with your local health board Health Disability Activity Practitioner to arrange. You can also get in touch with any other queries about the pathway or potential referrals.

Step One:
Patient contact with Healthcare Professional.
Then in Step two, the patient and their Healthcare Professional complete the Health Disability Activity Pathway Signposting Form.
Step three, the last step, begins with Local Health Disability Activity Practitioner screens referral. Then, either:
- Local Sport Development Team (or equivalent) will contact the patient or their parent/guardian to discuss the options available. or;
- Health Disability Activity Practitioner will contact referrer to discuss options available to share with the patient and/or their parent/guardian.
Options could include:
- signposting to suitable clubs and sessions
- helping patient get a Leisure Card to support access to local facilities
